(“Restore Health”, “we”, “us”, or “our”) in coordination with James E. Boyd, MD APMC (“Medical Practice”). By accessing, viewing, or interacting with the Site, you acknowledge that you have read, understood, and agree to be bound by these Terms and our HIPAA Notice of Privacy Practices. If you do not agree to these Terms, you must immediately discontinue use of the Site.
2. Corporate Structure & Medical Disclaimer
- Administrative vs. Clinical Roles: Medical services, clinical oversight, consultations, standing orders, and infusion treatments are provided solely by James E. Boyd, MD APMC, a California Professional Medical Corporation, and its licensed clinical healthcare practitioners. Non-clinical administrative, facility management, technical infrastructure, scheduling, and marketing services are provided by Restore Health and Wellness, Inc.
- Informational Purposes Only: All content available on this Site—including service menus, therapy overviews, clinical summaries, pricing, FAQs, and articles—is published strictly for informational and educational purposes. Nothing on this Site constitutes the practice of medicine or the provision of individualized medical advice, diagnosis, or care.
- No Physician-Patient Relationship: Browsing this website, submitting an electronic inquiry, or booking an appointment does not establish a physician-patient relationship. A clinical relationship is formed only after an intake screening and consultation with a licensed clinical provider.

4. Online Scheduling, Consultations & Cancellations
- Booking Requests: Online appointment reservations submitted through our digital booking integrations (including Acuity Scheduling) represent requests for clinical service and remain subject to final verification and clinical appropriateness.
- Cancellations & Modifications: Because clinical staff and medications are scheduled and prepared in advance, we request at least 24 hours’ advance notice for appointment cancellations or rescheduling. Specific appointment cancellation terms and deposit requirements will be confirmed at the time of booking.
5. At-Home Kits & Telehealth Services
Certain wellness items (including at-home self-injection therapies or compounded topical formulations) require telehealth screening and prescription authorization by our licensed clinical team prior to dispensing. All prescription fulfillments are handled in compliance with applicable federal and California regulations through licensed partner compounding pharmacies.
6. Intellectual Property Rights
All materials contained on this Site—including logos, typography, visual layouts, copy, graphics, button icons, photographs, and source code—are the proprietary property of Restore Health and Wellness, Inc. or James E. Boyd, MD APMC, and are protected under federal and international copyright and trademark laws.
