Telemedicine Consent

1. What Is Telemedicine?
Telemedicine (or telehealth) means Restore Health clinicians may communicate with you by phone, video, or secure messaging to conduct a health screening, review your intake information, or follow up on your care, without you being physically present with the clinician at that moment.
2. Voluntary Consent
Your participation in a telemedicine screening or consultation is voluntary. By proceeding, you consent to receive part of your care this way, understanding that in-person treatment (the actual IV infusion or injection) will still be administered by a licensed clinician physically present with you.
3. Limitations of Telemedicine
A telemedicine screening does not include a physical examination. Our clinician’s assessment is based on the information you report. There is a chance that some information relevant to your care could be missed without an in-person exam, and your clinician may determine that in-person evaluation is necessary before proceeding with treatment.
4. Right to Withdraw
You may withdraw your consent to telemedicine at any time without affecting your right to future care. If you withdraw consent, we will discuss alternative ways to complete your health screening.
5. Confidentiality
Information shared during a telemedicine interaction is protected under the same HIPAA privacy protections as an in-person visit. We use secure, reasonably protected communication methods; however, as with any electronic communication, we cannot guarantee against every possible security risk.
6. Technology Risks
Telemedicine depends on technology such as phone or video connections. Interruptions, poor connection quality, or technical failures can occur and may require rescheduling your screening or consultation.
7. Emergencies
Telemedicine is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately rather than contacting us through telemedicine.
8. Acknowledgment
By proceeding with a telemedicine screening or consultation, you acknowledge that you have read and understood this consent, had the opportunity to ask questions, and agree to receive this portion of your care remotely.
