1. Acceptance of Terms
By scheduling an appointment, receiving treatment, or using this website, you agree to these Terms & Conditions and the Medical Consent described below. If you do not agree, please do not book an appointment or receive services from Restore Health Infusions and Wellness (“Restore Health,” “we,” “us”).
2. Consent to Treatment
You authorize Restore Health’s licensed clinicians to administer the intravenous (IV) infusion, intramuscular (IM) injection, and subcutaneous injection services you have requested. Treatments are elective and are provided based on the information you disclose about your health history, current medications, and allergies. No specific outcome or result is guaranteed.
3. Assumption of Risk
IV infusions and injections carry some risk, even when properly administered. Possible risks include but are not limited to: bruising, redness, or swelling at the injection/infusion site; vein irritation or infiltration; infection; allergic or adverse reaction to a vitamin, mineral, medication, or additive; lightheadedness or fainting; and, for subcutaneous or intramuscular injections, localized nerve irritation or lipodystrophy (changes in the fat tissue at the injection site) with repeated use. You acknowledge these risks and agree to notify staff immediately of any unusual symptoms during or after treatment.
4. Medical History Disclosure
You agree to provide complete and accurate information about your medical history, current medications, supplements, allergies, and pregnancy/breastfeeding status prior to treatment. Withholding or misrepresenting this information may result in a treatment being modified, declined, or stopped, and you remain financially responsible for services already rendered up to that point.
5. Special Populations
Certain therapies may not be appropriate during pregnancy, while breastfeeding, or for patients with certain underlying medical conditions. Our clinicians will ask about these factors before treatment; please disclose them fully so we can determine whether a treatment is appropriate for you. Restore Health does not treat minors under the age of 18 without a parent or legal guardian present and providing consent.
6. Wellness Nutrients & Medications
Many vitamins, minerals, and nutrients used in wellness infusions are not evaluated by the FDA for the specific purpose described on our menu, and individual results vary. Where a treatment includes a prescription medication (for example, anti-nausea or anti-inflammatory medication given as part of an infusion), that medication is administered under the order of a supervising clinician.
7. Mobile / At-Home Visit Acknowledgment
If you book a mobile visit, treatment will take place at a location of your choosing (your home, hotel, or office) rather than in our clinical facility. You acknowledge that this environment is not a controlled clinical setting, and you agree to provide a safe, reasonably clean, and accessible space for your clinician to work.
8. Right to Refuse or Discontinue Service
Restore Health reserves the right to refuse or discontinue treatment at any time if, in the clinical judgment of our staff, treatment would be unsafe β including but not limited to signs of intoxication, an undisclosed condition that presents a safety concern, or abusive behavior toward staff. You remain responsible for payment for services already rendered.
9. Cancellation & No-Show Policy
We ask for as much advance notice as possible if you need to reschedule or cancel. Late cancellations and no-shows may be subject to a fee. Specific cancellation windows and fees will be communicated at the time of booking.
10. Payment
Payment for elective wellness services is due at the time of service unless other arrangements have been made in advance. Prices are subject to change without notice; the price confirmed at booking or at the time of service will apply to that visit.
11. Privacy & HIPAA
Your health information is protected under the Health Insurance Portability and Accountability Act (HIPAA). We collect and use your information only as necessary to provide care, process payment, and communicate with you about your treatment, consistent with our Notice of Privacy Practices.
12. Limitation of Liability
To the fullest extent permitted by law, Restore Health’s total liability for any claim arising from your treatment is limited to the amount you paid for the specific service giving rise to the claim. Restore Health is not liable for indirect, incidental, or consequential damages.
13. Governing Law
These terms are governed by the laws of the State of California, without regard to conflict-of-law principles. Any dispute arising from these terms or your treatment will be resolved in the courts of San Diego County, California, unless otherwise required by law.
14. Electronic Signature
By typing your name in the appointment intake form, or by signing electronically through our booking or consent platform, you agree that your electronic signature has the same legal effect as a handwritten signature.
